Practice Catchment Area

As a guide, the practice covers a radius of approx. 12km (8 miles), including Montrose, Hillside, Dun, Farnell, Lunan, Inverkeilor and Ferryden.

A detailed map of the practice boundary is available at both Grampian and Tayside Health Boards’ Primary Care Department and can also be viewed at the surgery.

How to Register as a Patient

All new patients are required to complete a new patient registration check, this will be sent out to you from the practice by SMS and will have a link to a medlink new patient registration form.

You may need to attend an appointment for a blood pressure check, bloods or other diagnostics, this appointment will be at the care and treatment centre within Links Health Centre or there is a self check machine located at Annat Bank Practice.

Non-urgent advice: Please Note

You can only register with a GP practice if you are staying in the area for more than 3 months. If you are not, you may still be treated, but as a Temporary Resident.

You can also register for online patient services to order repeat prescriptions.

To be completed in full. We request proof of identity for all new patients registering; this can be in the form of a photo driving licence, passport, bus pass etc.

Parent’s identification is adequate when registering children aged under 16.

New Patient Questionnaire

We require information which will be held securely in your medical record.

Please provide as much information as you can.

Online Services Registration Form

Register for online services to order repeat prescriptions and book certain appointments online.

Non-urgent advice: Please Note

Until you are fully registered with the practice, you will remain registered with your current GP Practice.

Until we receive confirmation of your past medical history and medication, we will not be able to issue any repeat prescriptions. You should ask your previous GP for a supply until your registration process is completed.

It is important that you complete and return all forms including identification (ID).
